Bangalore: One-year rural service will be mandatory for doctors, says Khadar

Bangalore, May 23: Karnataka’s new Health and Famil Welfare Minister U T Khadar has announced that the State Government was committed to make one year rural service mandatory for all MBBS and medical post-graduates studying in the State.

Khadar said a Bill pertaining to rural service was pending before the Rashtrapati Bhavan.

Both houses of the State legislature have passed the Bill during the tenure of the BJP government.

''We are committed to enforcing the Bill as soon as it receives the assent from the President,”  he said.
Doctors will have to shell out hefty fines if they failed to serve in rural areas.

Under the provisions of the Bill, MBBS doctors will be fined Rs 15 lakh, the PG diploma and PG doctors will be fined Rs 20 lakh and Rs 25 lakh respectively, if they do not serve in rural areas for one year after completion of their course.

A delegation would be taken to Delhi to urge President Pranab Mukherjee to give his assent to the Bill soon, the minister said.

The Government will set up an Equipment Monitoring Cell in the State to monitor functioning and repair of all existing equipment in primary and community health centres, taluk, district and state-level hospitals.

Malfunctioning equipment would be repaired only through companies that manufactured the product. The Cell manned by three officials would get the defunct equipment repaired in 24 hours.

A technician attached to the health centre or hospital has to inform the Cell about the malfunctioning of the equipment and reasons for its disuse.

After noting down the reasons given by technician, the Cell would inform the manufacturing company to repair the equipment, he said.

A large number of equipment purchased for use in x-ray, ICU and operation theatre were not being used either to neglect or malfunctioning.

To ensure its use and provide quality service to patients visiting hospitals, the Cell would be set up to monitor functioning of equipment, Khadar said.

In future, equipment of only reputed manufacturing companies would be purchased through tenders.

The role of middlemen in tenders or procurement of equipment would be totally avoided, Khadar said.

The government was mulling on formulating a general health insurance scheme for above poverty line families in the State. The government would reimburse bills for surgeries and other treatments, Khadar said.

The officials have been told to work out the plan. Already, there was a insurance scheme for BPL families.

Under the Arogya Kavacha scheme of the government, a decision has been taken to extend the state-run 108 ambulance coverage beyond 80 kms. Now ambulance provides emergency response service within 80 kms only.

Permission would be given to 108 staff to take the patient to hospital located beyond 80 kms if there was no hospital with the 80 kms radius, he said.

Now one ambulance was deployed for a lakh population. With the increased demand for 108 services, an ambulance would be provided to 50,000 population, he said.

The Government spends Rs 221 crore for implementing the Arogya Kavacha scheme per year.
It receives Rs 80 corre from National Rural Health Mission, he said. 

Started in November 2008, the emergency service run by the government in association with GVK Emergency Management and Research Institute (EMRI).

Under the NRHM, the government would receive Rs 1400 crore in 2013-14 against Rs 1062 crore in 2012-13.

The State would receive Rs 34 crore in the current fiscal year from the Centre for taking up non-communicable disease programmes.

Under the Urban Health Mission, the Centre has sanctioned Rs 104 crore and it would be taken up in Bangalore only, he said.

He said the Director of Health has been directed to submit a report on number of persons affected by endosulfan in the State.

A district-level committee headed by District in-charge minister would be constituted to provide health care facilities and monthly honorarium to endosulfan victims, particularly in Dakshina Kannada district.

Providing one-time compensation to victims would not solve their social and economic problems, he said adding: ''The victims need permanent solution.”

    Before doing this mandatory, all the rural hospitals should be equipped with basic infrastructure and other medical facilities including supply of medicines, injections etc. Otherwise whats the use of posting the doctors to rural places. The very purpose of this will be defeated. Our Health Minister should bear this in mind and try to have this.

    Congratulations. I had met your father when I was a high school student in Bank Of Baroda, Mangalore. He was a simple, humble man.
    I agree with your plans of rural service for physicians. We need to consider certain issues.
    1. Provide decent accomodation/safety for all doctors especially women.
    Provide transportation/electricity/water/internet access etc...let it not be a punishment.
    3.Provide reservation in PG seats for doctors who do rural services.(I think St Johns Medical College follows this practice).
    4. Let there be enough work which helps in learning new things, not just treating cough, cold.
    5.Specialists like radiologists might not have work/infrastructure.
    6. Educating people about blood pressure, Diabetes, Cholesterol, Cancer prevention,weight loss healthy diet and exercise and thus prevention is better than cure should be implemented.
    7. Provide clean drinking water, clean air and toilets to rural population.
    8.Please involve Dr Devi Shetty in decision making in providing health insurance to all so that if anyone needs expensive treatments like heart surgery/dialysis they are covered.
    9.Educate youth to not to smoke and plans to quit smoking.
    10.Last but not the least let Mr.Siddaramayyaji(only if you are comfortable) know that Alcohol does more harm not only to the health but to the society.

    Good cause but wrong means.

    1. What about students from private colleges who pay the fee for the service - here education - in full. I can understand this for students who avail government grants (e.g., through subsidized fees).

    2. Community service for doctors should be optional and encouraged, not enforced. We aren't the subjects of a communist regime, are we?

    3. to the esteemed readers: How many of us are willing to spare one year of our life for community service, say immediately after graduation. Very few I guess.

    if people keep neglecting rural areas, then there would not be anybody there. Just to not hurt you, govt should increase the salary of the people serving rural areas and reduce the salary for city doctors. Then automatically most of them would serve in the rural areas for the sake of money....

    Couple of days back Mr.Khadar visited Dharmasthala Dharmadikari Sri Veerendra Hegde.When Minster asked Sri Hegde to give some tips for the development of rural areas,he requested him to impliment new bill where every doctor should work in villages for some time.But even in my dream,I never expected that our Minister would impliment Sri Hegde's advice so soon.Combination of Sri Hegde and Sri Khadar is good.Congrats..

    First of all, the quality of doctors produced by the dozens of mushrooming private medical colleges leaves a lot to be deserved, couple that with the forcible postings of these people, mostly from outside our state, will in my view go against the poor gullible rural patient s interests. Also, why the doctor is only singled out for this service is hard to fanthom..he spends atleats 10-15 years in medicine already before he starts earning ! The other scapegoat govts find r teachers..required for census, voting centre everywhere..why not RTO official s income tax dept etc ?

    first of all make d rural infrastructure better than ask d doctors to work there...i think d minister thinks all d hospitals in rural areas r lik aj or kmc...govt medical colleges produce 100s of ug n pg doctors who take benefit of reduced fees but where are these students working in d rural areas?? proper infrastructure, proper supply of medicine, adequate para medical staff, security, needs to b ensured and well pay according to d new scale

    I am surprised to see some congi agents comments. When previuos health minister proposed this, they blamed him saying it is personal choice of doctors and minister should not interfere.

    I supported this When BJP minsister proposed this. Even Today I support UT Khader for this decision.

    Please keep an eye on the medicines supplied by some gujarathi(Marwari)companies.Appoint a cell for that also to monitor the (substandard or no standard at all)drugs supplied.B-complex capsules just contains wheat flour!
    Go ahead.Do some sudden ,surprise visits to drug ware house.You can see the middlemen wandering there.

    Why is this rural service only for MBBS and MD students? Why not also for other graduates and post graduates? Even those doing MSW and MBA, for example, can offer their skills in planning communitarian development and management in rural areas.

    most of the politicians until their party gets elected they perform walking/jogging march (pada yatre) around all the rural areas for their future gain. However, Khader Anna is different. He is for all the people rural or town . However, he also wants to take doctors with him for some pada yatre for the rural people's gain. That is fine.
    But next time can some of the Doctors CAN GET THE TICKET TO CONTEST IN THE ELECTIONS for their self-less/hardship service for the rural mankind ? or MLA Anna will keep on gaining points at somebody's hardship labour?
    Therefore, in general elected MLA's or MP's must be entrusted upon rural assignments to spend some of their precious political life at the grass root of democracy , and should not be meant for Doctors's, Engineers, Teachers, Nurses, etc., etc., all alone..

    This mandatory service will instill patriotism, humanity in the newly graduated doctors.

    I would also like to see the Indian govt. initiating a one year mandatory military service to all able bodied youth, in the similar fashion and goals.

    Good measure by Khader for preventing corruption in procurement and maintenance of medical equipements that result in better services to the patients.
