Mangaluru, Sept 21: A team led by Dr Anand K T, cardiothoracic and vascular surgeon, and Dr Ariharan K, interventional radiologist, at Father Muller Medical College Hospital has successfully performed a complex endovascular repair of an abdominal aortic aneurysm (AAA) in a patient aged over 80, marking the third such complex endovascular aortic procedure performed at the institution this year.

The patient, aged over 80, had an abdominal aortic aneurysm associated with significant underlying kidney disease. The patient’s advanced age, renal dysfunction and markedly tortuous vascular anatomy made the procedure particularly challenging and required detailed pre-procedural planning and careful intra-procedural management.

Advanced endovascular treatment

The patient underwent total endovascular aortic repair using specialised aortic endografts, allowing the aneurysm to be treated through a minimally invasive endovascular approach.

The procedure was performed in the hospital’s advanced biplane Cath Lab, which provides high-quality, real-time imaging from multiple projections. The facility allows precise navigation of endovascular devices and accurate deployment of complex vascular grafts, particularly in anatomically challenging cases.

Due to the patient’s complex anatomy and medical comorbidities, the procedure required meticulous technique and lasted more than seven hours.

Anaesthesia was managed by Dr M M Chetana Anand, consultant cardiovascular & neurovascular anaesthetist. Given the patient’s advanced age, renal dysfunction and the prolonged duration of the procedure, careful anaesthetic planning, continuous haemodynamic monitoring and meticulous peri-procedural management were essential to ensure patient safety throughout the intervention.

Despite the complexity, the procedure was successfully completed, and the patient made an uneventful recovery. The patient was discharged in good condition three days after the procedure.
