Mangalore: A Month After - No Report on AIE Crash

Mangalore, Jun 22 (TOI): Even a month after Air India Express IX 812 crashed at Mangalore airport killing 158 passengers and crew members, several important questions remain unanswered. The civil aviation ministry and the Directorate General of Civil Aviation (DGCA) are not keen on releasing a preliminary investigation report. It points to a lack in transparency in the system. TOI chalks out a list of five questions which need to be answered:

Why doesn’t India have a practice of releasing a preliminary investigation report after an aircrash?

The Digital Flight Data Recorder (DFDR) can be downloaded in 15 days and the Cockpit Voice Recorder (CVR) can be decoded in a couple of hours. Both of these were retrieved just days after the Mangalore crash. A court of inquiry headed by Air Marshal Bhushan Nilkanth Gokhale, former vice-chief of air staff, and four others will investigate and submit the findings by August 31. According to sources, the inquiry team sought a deadline extension and it has been granted.

The last airline aircrash investigation report to be released in India was that of Alliance Air Patna aircrash in 2000. After three years, a page-long investigation report stating pilot-error as the cause of crash was released.






Why doesn’t the investigating team have a single official who is an expert on the particular aircraft?

The aircraft which crashed was from Boeing 737 Next Generation series. Airline accident probe in India are always handled by IAF officials, who are not familiar with civilian aircraft types. Of the four investigators, Capt Ron Nagar is the only expert with experience in civilian aircraft. The country does not have an independent agency like US’s National Transportation Safety Board or the UK’s Accident Investigation Board.

Why was Mangalore airport’s only radar, the Air Route Surveillance Radar (RSR), kept off?

Why did the Mangalore airport authorities need 72 hours to carry out monsoon maintenance work on the Westinghouse RSR when the same is done in 12 hours in airports such as Chennai and Guwahati, which have a similar radar from the said company? If the radar has been on, the controller could have seen the aircraft’s height and if it was high on approach, the radar controller would have warned the pilot for altitude correction.

Why was an outsider (a villager) and not an air traffic controller, the first person to inform the police?

Air traffic controllers are trained for emergency response, but manpower shortage makes it impossible for the controllers to inform 20 other units within the desirable 5-10 minutes of the accident. How long did the fire tenders take to reach the scene?

Why is Air India still following lower ‘G’ limits for landing, even after DGCA has issued a circular to all airline operators to maintain Aircraft Maintenance Manual (AMM) limits for landing?

Weeks after the tragedy, the DGCA issued a circular stating that pilots need to be made aware that achieving a particular G-value (vertical acceleration due to gravity on touchdown) is no measure of a good landing. Landings should be judged not by how soft the landing has been, but if it has been made at the correct speed and touchdown zone on the runway.

    One of the ways to get the report out is by putting a complete stop on traveling by AI flights to any of the destinations. This is possible only when we all unite as one and fight for justice. Are we ready for that??

    This is one of the incidents that will never ever be erased off our minds. Our tears are still rolling out when we recall that scene.

    Why anyone is surprised that there is no report yet although 2 months have passed! This is INDIA. Our caliber is that.

    Don't seek justice. Just take it on you & punish the bureaucrats! collect 1 Lakh contribution from each compensation & I guess you can get justice the way, taking law in your own hands to put the souls to rest!

    One of my friends relative's husband was American national and was working for the firm where the black boxes are made, he used to tell them that the black boxes are made, in such a way that after the crash they can find out from anywhere they are and easy to find the cause for the crash.

    Where is the black box? in US??? forget it from geting results....... if we cant extradict Anderson, Rana & Hedly or Idly from US how come we can expect the reports from US based co who is manufacturing such air crafts????????

    Please consider Time taken for Bhopal Judgment as our Indian Standard Time for any report delivery or to act on any issue. Do not take tensions as it is not going to change us. May be next generation may be reading the report in this case, if given one. We are not as precious as Americans.

    Well in most parts of the world including the advanced countries, it takes atleast a good 6 months for a person to come up with a descent report. In this case it’s INDIA so it may take at least 6 years. In some cases where officials involved are honest and care for the inncocent who lost their lives, provide a honest report, exposing the flaws of individuals involved in the mishap. Although in most cases they sweep it under the carpet to prevent opening a new can of worms. Considering that this happened in India, and to add to it with the government aviation body of India which is named “Air India”, more so we have a government appointed official performing the investigation and would eventually come up with a report which the government would have a first hand look at. There is less than 1% chance of getting a honest report and a unbiased version of the events that led to the mishap. It would be more like a cover up act than a corrective measure report. The persons who are unable to present their part of the story is the deceased pilots so the easiest thing to do is to place the blame on the dead as the well know proverb goes – “A dead man tells no tales”. Sorry for the all of you who lost your loved one’s. If we expect the governement official irrespective of which party they belong to understand us it would be a dream that may never come true. The governement body in India knows only one language and that is named “CORRUPTION”.

    Yaaaar... Indian Government atleast need 25 years to take corrective steps in every dissaster.. Look at Bhapal, Look Haryana Ex-DGP Molestation case...so on and so forth.. so surely in this Crash matter too after 25 Years a report will come-up and Media will encash through Public outcry... then Government will form GOM (Group of Monkys or Group of Ministers) and then some releif will be d... and end of the story.. this will continue !

    Air crash investigations usually take years dont expect any report sooner. Have you seen the program "Air Crash Investigations" in National Geographic Channel???. Even countries like USA have taken years to conclude some investigations.

    EVERY INTERNATIONAL AIRPORT RECORDING VIDEO OF EACH LANDING AND TAKEOFFS. IS THERE ANY CAM / CC TV AROUND IXE AIRPORT ? EVEN AFTER ONE MONTH ?! (Is there any Rs 6-5k Digital Camera ?! )

    WELL SAID ASHOK UDUPI.
    Bhobal like incident can occur anytime in India. Even after 25 year have not learnt anything.

    I strongly feel, that unlike any other country its only in india that things are stretched for years together and by the time the reports are released the next of kin is already dead. Including our national carrier AIR INDIA has got no standard of service and thus none of indian pilot are willing to work for this airline. Prefer to travel by private airlines, which are much efficient and service oriented. We talk about AI being going on loss.. Naturally it will be on loss if the aircrafts are not being maintained well and results in air crash. So as the airports too.. India need to think in this issue now and support smaller airports by giving them required facility and equipments. AI can still run a good show, subject to they change thier attitude and way of working. Feel sorry for those people who have lost thier lives on May 22 air crash at M'lore. My heartly condolences to their families and may god rest the departed soul in peace.

    Everything is slow in Mangalore. look at NH up gradation. its now more than 5 yrs still 50% of the work is not completed dat is too 40 kms of highway........new terminal has taken nearly 2 yrs to inaugrate but aftr inaugration yet operation has not strtd.........der r many examples.....this quiet common in mlore

    The outcome of the report is mostly predictable - PILOT ERROR! But how come an experienced Pilot can commit error? It's because of improper aviation system of India. Pilots have tremendous pressure to soft land a plan as airlines insist for the same for the comfort of passengers. But highly risky as in the case of Mangalore. So if the pilot takes risk and lands safely, appreciate airlines. If the pilot crashes a plane, nail him if he is alive OR blame him if he is dead!!! Then where does the experience of the pilot count??? God please save Indian aircraft passengers!!!

    Daijiworld world, appreciate your gutsy approach. Well, not sure how many of them can survive if they are up to produce such a honest reports.
    About DGCA let them take another 3 to 5 Years as it looks very much a short time when compared to some of the cases that takes 25 and more years to finalise!!
    For those who have already lost their best ones are for ever and the memory remains too for ever. For the money building politicians it’s just matter of another gamble.
    What is the Law and Order in India!!! £"£"£!"£ question!!.

    It is strange that the investigators in our country takes so much time to investigate the problem, what had exactly happn on the flight?The technology is so advance that it still takes 3 to 4 years to find out the problem / reason of air crash. It should hardly take 15 to 20 days to find out the exact reason. The staff should be appointed to investigate in different field at the same time. The report should be gathered and then publish. So the reason will known quickly. Other wise atleast a brief explanation on the reason of air crush. The best is the national geographical channel on air crush subject. If India is having latest technology it would have been favoured better. Any ways hoping the result and reason for crush will be released ASAP. India is GREAT

    Its very clear, when there is no good governance, there is no responsibility, and there can't be accountability. This is also the main route cause for all the problems what we have in India today. Development of under previlaged , Naxalism, Basic infracstructure, Ethics driven society, Value based education ...all all are only because , our PM and his team thinks on Inflation, GDP, and Planning. Stop all this and concentrate on Implimentation of all those Plans. Revolutionalise the Education system. Let there be a goal setting and evaluation of the system . If failure let the Minister denied 10 years in Polictics. Set some hard base rules., or else the Monkey mind in the Minister , Beurocrats, Union leaders, Industrialist will prevail.

    It is unfortunate that we are decades behind in many fields and in certain matters.Probably the authorities want that everything should cool down first and no one should be anxious about the report. The charred dead bodies have been valued a few lakhs, but who will value the human beings-I mean:mothers,fathers,brothers,sisters, sons, daughters,nephews,nieces,husband, wife,child.........-except those who still mourn for these unfortunate?...those who still wait for their dear & near ones to reach home?... those whose eyes are still longing to see their beloved?... and who still believe that they are living?....without getting reconciled to the fact that this unfortunate incident has ever taken place...and those still living in false hopes? It might take months together or year to bring out the report to the general public or probably that the authorities might wait for a miracle of the pilot's resurrection to tell us what exactly had happened.What is gone is gone, can the saviours of millions of lives save what could be possibly lost in the future?

    And about air traffic controller may be at that time he moved out for tea break or something….!!!

    Can the investigators look into the landing details of the same flight on 9.5.2010 (two weeks before the tragedy) if The flight had overshot the runway on that day also. If yes, why the necessary actions were not taken?
