Dear doctor,
I think I have a serious heart problem. I have consulted many cardiologists but they say everything is fine. I get chest pain, increased beating of heart and breathlessness. I have been having this since 6 months. I lost my job 1 year back. I have no children. My husband works in Dubai. I have returned to India to look after my mother who had developed stroke 6 months back. Do u think I have a psychiatric problem?
Sharol

Dear Sharol,
Last year has been very challenging for you both at work and home. You have additional responsibilities and stress of migration looking after an unwell person and staying away from partner. All this can cause significant emotional distress. You have described symptoms which occur in anxiety disorder. There is a possibility that you are suffering from hypochondriacal disorder. Consulting a psychiatrist will definitely help. Please carry your cardiology reports when you visit the doctor. The first thing that we do is rule out physical conditions that mimic anxiety. All the best !

Dear Dr Supriya,
Greetings of the season! My aunt has a peculiar problem. She gets crawling sensation all over her body. This has been happening since 6 months and we have shown skin specialists. She believes there are insects under her skin. She can hear them moving around and gets burning sensation where they lay eggs!!We took her to a psychiatrist also. But she didn’t take medication properly. She is constantly complaining and keeps scratching. She is a widow. Her children are in Canada. What to do next?
V

Dear V,
Greetings to you as well. Your aunt has a condition what we call as delusional parasitosis . In this disorder the patient believes that she actually has insects in her body. It has to be treated with antipsychotics and psychotherapy. Some creams and emulsions may be required to treat skin excoriation caused by scratching repeatedly. She will require long term treatment. If medication is stopped early, the condition will relapse.

Dear Doc,
My problem is that I cannot concentrate. I lose focus easily. I get bored easily. I don’t have any permanent interests. I am totally excited once and totally bored soon after. I was a good student in school, played football. I tried stimulants in college. I am addicted to coffee. Problem is I think I have adult ADD. How do you decide if I need treatment or I can control it by myself?
Rahul

Dear Rahul,
You have symptoms of adult ADD. There is a possibility you had many symptoms from childhood itself. Childhood history is best got from your parents. The severity of the condition and the socio-occupational dysfunction it causes, helps us decide if medications need to be combined with therapy or therapy alone will suffice to help you get better. Controlling it yourself with self-help books may not be advisable as you may get only general advice and not one specific to your problem areas. My suggestion is that you consult a psychiatrist and clarify your doubts personally.
