Udupi, Feb 2: In an incident that has come to light, a man, used fake signature and created fake bill, discharge summary of a hospital in the city and submitted the same to claim insurance in an overseas country.

According to details, first accused Jahinaab Muzafar got treatment as outpatient at the CSI Lombard Memorial Hospital on August 3, 2019. After that, he did not visit the hospital at all. However, he joined hands with second accused Muzaffar Ali Mohammed Sharif, third accused Lubna Banu and fourth accused Purkhan Muzaffar and used fake signature of the doctor and submitted fake bills and discharge summary to Bhoopa Arabia for Cooperative Insurance Company which is situated overseas.

The overseas insurance company sent the bills and discharge summary to verify the claim. The hospital authorities quickly noticed that the accused have forged the signature of the doctor and also made fake bills and discharge summary. Administrative officer of the hospital Deena Prabhavati has filed a complaint in this regard with city police station.
