Bangalore, Feb 7: Shirish, three-year-old son of K S Lokesh, who owns a chain of hotels in the city, and residing in HSR Lay Out here, and  his wife, Asha, reportedly lost his life within minutes of being injected with anaesthesia at a dental clinic in the city on Wednesday February 6.

Asha had taken her younger son, Shirish, whose third birthday the family was to celebrate on March 12, to Annayya ENT and Dental Clinic at fifth sector of HSR Layout as he had complained of toothache the earlier night. At around 8.00 am, Asha said, Dr Raviraj of the clinic administered anaesthesia to do root canal. The child became unconscious within minutes thereafter and remained so even after three hours. The doctors kept assuring the mother that the child would wake up shortly.

After waiting for long, the doctors of the clinic took the boy to Garden City Hospital, Jayanagar. The doctors of this hospital tried to revive the child, but did not succeed.

Relatives then beat up dentist, Dr Raviraj, and anaesthetist, Dr Raghavendra, at the Jayanagar hospital, alleging medical negligence on their part, and ransacked the clinic premises. They claim that Shirish had breathed his last of cardiac arrest within seconds of administering anaesthesia, but that the doctors had tried to conceal the truth. They also said the action of the doctors in taking the baby to Jayanagar instead of nearby St John's Hospital has also proved that there was foul play.

The body was handed over to the parents after conducting post mortem at KIMS.

A case of medical negligence in this connection stands registered in Madiwala police station here, against the doctors and staff of the hospital. Asha said that she had reached the hospital at 7.30 am, and Dr Raviraj, at around 8.00 am, told her that root canal procedure would have to be performed on the boy immediately to ease pain. She saw anaesthesia being injected into the boy's mouth, and shifting of the boy to the ward by the doctors sometime thereafter.

"To my repeated questions on the boy being shifted to the ward instead of being treated in the clinic, neither the doctor, nor the hospital staff provided satisfactory answers. I was not allowed to see the boy, but the behaviour of the people there suggested that something was amiss," she said.

She said Dr Raviraj and others administered more injections to the boy, who was not moving. Only at around 10 am was she told that the boy had suffered cardiac arrest, and that he was not responding to treatment. She said when she insisted on knowing the problem, the doctor told her that there was something wrong with her son, and that he would be shifted to a hospital. She panicked and called her husband, she explained. She said doctors at Garden Hospital had a hushed talk with Dr Raviraj, but told nothing to her. At around 1.30 pm, she was informed that her son was no more.

The police said they will wait till the post mortem report is received, and take action only depending upon its findings on the cause of the boy's death.

Dr Raviraj said he had done nothing wrong, and that vomitting and then swallowing it back on the part of the boy might have caused cardiac arrest. Dr Dhanraj, brother of Dr Raviraj, said that the boy suffered heart attacks thrice on Wednesday.

The doctors have also filed complaint of assault against the concerned.

    Its really sad to hear this news, I'm studying for dental profession. I took this course thinking that no patient can lose life by a dentist's hand as they are just handling with the tooth. But, after hearing this, I feel studying dentistry isnt a silly matter(I mean, I didn't opt for medicine in fear of losing patients life or something harm hapening to the patient. I felt myself not eligible for it. Felt bds just deals with restoration or tooth extraction. But to be frank, dentistry is a beautiful field :) . Its really more than what we think).

    I hope this child's incidence awakes the future dentist who just thinks that as it was in school by-hearting everything just for the sake of pass mark or good mark is enough and the answer for asking why to become a doctor is to get money rather than giving value to the patient's money.

    Hi. I myself is specialist in dentistry. Sad for both patient and doctor. But dentist should never take undue risk unless clinic is highly equipped to handle emergency. Both dentist and anaesthetist should know the seriousness of life. If it is their child I don't think they would have handle like these.

    I'd say the child had lived its life for the span of years assigned to it.! Sad that it was just three.! For unqualified people to blame the death on the Doctor is foolish.
    Some person has brought up the issue of management quota for admission to medical degrees. Well son, that quota is only for admission. the student has to answer and pass exams like everybody else and score over 50% to pass. Medical education is still one of the toughest. If the medical institutes were not charging so heavy for a few seats that they sell they would not be able to afford good teachers and infrastructure and we would have bad doctors.! Charging money for admission is good. people who have the money will spend, who dont will get get in through merit, but once they get in its all pretty much the same.!
    And its a senior Doctor we are talking abt in this case.

    It is sad to note the tragic demise of a young soul after dental treatment and subsequent unlawful beating of a dentist. It is very difficult to comment unless the postmortem report is out. However, it is more depressing to read many so called ‘literates’ in this forum questioning and commenting about the need of saving a milk tooth in a 3-years old child. As we live in a land of superstition, everyone thinks that he/she is the master of all!! It is pity that people still believe a bullock-cart driver, if he boosts about techniques of riding an aeroplane rather than listening a qualified pilot!!
    It is a known fact that, Baby teeth (milk teeth/primary teeth/deciduous teeth) are necessary for chewing, speaking, and smiling. They also serve as placeholders for the adult teeth. Tooth decay in infants and very young children, often referred to as ‘baby bottle tooth decay’, which can occur when sweetened liquids, milk or even sugary solids cling to an infant or child's teeth for long periods. Giving a child a sugary drink at nap or nighttimes is particularly harmful, as the flow of saliva decreases during sleep. Bacteria in the mouth thrive on this sugar and produce acids that attack the teeth. If tooth decay is left untreated, pain and infection can result, which can be life threatening too!! If teeth are infected or lost too early due to decay, the child may develop poor eating habits, speech problems, crooked teeth, and even damaged adult teeth. In addition, the chances that adult teeth will end up being crooked are greatly increased. That is why it is wise to save the decayed milk tooth until they are ready to be lost natural until they are ready to be lost naturally. For e.g., in the case of molars it should be saved until around 10-12 years of age. With sound clinical techniques and some rather extraordinary compounds, a lot can be done to save decayed baby teeth, by either filling or root canal treatment. As always, treatment is based on assessment and diagnosis and especially in the case of milk teeth, may be decided by the state of the tooth at the time of examination.
    However, as you know prevention is better than cure. Parents should make sure their child's teeth are brushed and flossed daily and limit intake of sugary drinks and foods. The following things one can keep in mind to reduce the possibility of cavities in kids:
    • Wipe the baby's gums with a clean gauze pad or cotton after each feed.
    • As soon as the teeth start to come through, you should start brushing them. You can buy special soft brushes for babies and make sure you use a fluoride toothpaste
    • Clean and massage gums in areas without teeth.
    • Floss once all the baby teeth have come through.
    • Take your child for regular dental check-ups.
    • Never allow your child to fall asleep with a bottle containing anything but water.
    • Decreasing your child's sugar intake, especially between meals.

    Dr I completely agree with whatever you have mentioned, but all dr raviraj had to do was take him to green view hospital that was next door. As soon. As he saw the anaphalactic reaction he could have been rushed to the hospital. Why did he not do that. And the parents were not informed about the anaesthesia..be it local or conscious sedation. Thatsbunethical and cruel .

    Very sad, our sincere condolences to the fly. Last yr, my brother was to go for an operation for pancrea problem in M'lore in a big hospital, after anaesthisa for abt 6 hrs he was unconscious, then they postponed the operation for the second day, thank God he was saved!

    Doctors are not God, As they cannot see inside the body of any patient without CAT Scan or MRI or through proper diagnosis such as medical tests, If something was wrong inside the body of the patient. Profession is achieved only through education and experience is achieved only through practice. Doctor can identify and diagnose only through proper Medical equipment and medical tests. A doctor cannot identify what is wrong just through his/her eyes, hands, and ears. The doctor may have done tests before anesthesia induction. If anybody here is perfect and have not done any mistakes in their life or profession are born as God. There will be a postmortem and there will be findings and there will be a medical report. If there was any medical error then there will be a penalty according to the Indian law. Beating the doctors and staff is not right as it is an immature behavior of the patient family and relatives. When there is a death caused by some reasons which is not natural of course there will be anger within the family, relatives, friends, and community. But we cannot take law into our hands as it is also a crime to hurt somebody. There is always a bond to be signed by the patient or by the patient's parent or parents before the procedure or any surgery is performed or done as there are other risks involved.

    I know this doctor raviraj (senate member of RGUHS), he recently stood for Rajeev Gandhi University senate elections and won with highest no. of votes, for the same he had spent huge amount of money. To recover the same, he was know to charge his patients hefty amount money. Its clear that the doctor was money driven.

    profession, he is a murderer.
    How many such killings are taking place through killing people due to unwanted treatments and operations, simply to make fast money? The no 1 killer in the U.S. is heartattack, 2nd cancer, and the 3rd is due to medical negligence and unwanted operations
    and prescriptions of deadly drugs.

    well...to begin with.....i think this is just one another example of media blowing up a case without showing the true picture of both the sides....i hv my deep condolences with the grief-sticken family...

    But i wish to say is......m myself a pediatric dentist...and want to clear out few things on that front...

    1) its absolutely rubbish on common people part to claim that its a crime to do RCT on milk teeth....one shud be aware that pediatric dentistry is full specialized branch which is at par what pediatricians do to young children...every day...there r thousands of children being operated under GA for both medical n dental reasons....and its as much safe as LA...provided proper precautions hv bn taken...

    2) secondly i think, there is sm missing link in report....no where it hs bn mentioned that child was intubated or anesthetized with GA.....one more rationale is that one needs to prepare for it in one day advance with proper pre-anesthetic check up....

    3) n in case this is just a local anestheia, then it mite be a case of local anaphylaxis....and it cannot be a overdose...as it is given in small amount as 0.5 -2 ml.... watever...the reaction ws more of emergency type which could be due to any congenital condition too...which is extremely difficult to diagnose on a 3 yr old child with its first dental visit with no medical history.
    The same thing could have happened anywhere else with any anesthesia administration....

    although, this medical emergency could have been managed....and m sure...d doctor there mus hv tried its part....bt tragically...it wsnt enuf..

    I know its hard to make up for the loss....and doctors r alleged of being insensitive....but no one can appreciate kids as much as we pediatric dentists do....as our whole day is spent with them.....

    Dentist has made the right decision of saving the tooth by root canal treatment(pulpectomy). infected molar should not be extracted unless child is atleast 10 yrs old inorder to avoid future problems associated with loss of space.
    But since it was childs 1st dental visit, dentist should not have performed treatment and kept the appointment short by just taking a proper history to know if child is medicaly fit for the treatment and by prescribing syrup for the pain to know how the kid responds.

    This can happen at any age to any of us... we cannot blame anyone. one in every 1000 people are allergic to anaesthetics, penicillins, few other medicines, seafood, mutton etc. even a small dose of these can cause death if the person is allergic to it. Most of them fail to inform the dentist regarding their medical condition thinking itz just a matter of tooth.
    In India Dentist are not trained to manage a medical emergency.... Only theory knowledge will not help while practising. Dentist should have shifted the kid immediately to a nearby hospital rather than delaying for 2hrs.

    Doctors are said to be servants of God. But now a days some doctors have become servants of Satan. good comment. when these doctor STUDENTS or their parents pay 50 Lacks to the collecge for an admission the college administers says this is not enough. Other person is ready to pay 55Lacks/60 LACKS - some for PG COURSE - 1 core or nearby. When they spend these money and study their 9 OR MORE years what morals they want to show to the college or the public.
    These are the fault of the beurocrates who do not show SYMPATHY/humantrain grounds to the students who wants to be doctor. Beating the doctor is not going to solve the issue - Beat starts from the Medical councile of india to the college - who collects such a lot of money for this profeesion - why MCI cannot allow more seats to Medicals college and particulary for PG course - why the concerened authorities NOT CHECKING what amount collected from each student and why they close their eyes when they get their treats in cash or kinds. so end of the day this will be happend to every one. so wake up and save this problems. MEDICAL PROFESSION IS A SAINTLY PROFESSION TO SAVE THE LIFE - SO TEACH THE COLLEGE ADMINISTORS AND MCI ADMINISTORS TO PROTECT THE PROFESSION. BEST LUCK

    doctors you call it anything overdose, root canal needed or no.\? this is secondary. who will replace this childs pplace in the family?anaphylactic shock can occur with anybody with whatever the dose may be. but there is a antagonist which can revive the patient. If you do not have emeregency drugs and qualification to deal with emergency do not open the clinic. secondly why did you hide the fact from the mother?for more than 2 hours. if that was your baby will u keep quiet for 2 hours? were you making a patch up story to tell the parents? anything first discuss with the parents. secondly why did u file a cse for the assault? They are out of their sense when they lost their little darling. Even if the child had to go for general anesthesia isn't it the doctors resposibility to keep the patient fasting minimum of 6-8 hours? why did you jump int conclusion for the root canal treatment. There is no conservative treatment available? reduce the pain, discuss the treatment with the family, ask the past and present medical history. if necessary for the anesthesia take a written consent. If u want the money anyway this is what will happen to you.

    First of all I wonder, will any doctor perform a root canal on a 3 year old child ? it is very much evident that this dentist did know nothing on root canal. As per dentisry root canal can be done only on such persons, who are cable of bearing the pain and minimum age for this surgery is 15 yrs as they are quite strong to bear the pain and the hitches during the process. So sad that a innocent life is been taken away by the doctor. Dr. Raviraj should be punished under criminal act.

    A clinic must have basic equipments for any kind of emergencies. Its high time we set up a standard in India.All the doctors and nurses should be taught how to deal with emergencies. Apart from all these things, one should not take things for granted.     Early childhood cavities is become a big problem in recent times, our diets have changed but awareness among parents is low. Prevention of cavities is given least importance. Parents go to dentist only when the child suffers from pain. Because of difficulty in performing any kind of treatment in younger children, treatment under nitrous oxide gas or general anaesthesia becomes the last option specially when the child is in pain. This has its risks, most common as seen in this case is vomiting which enters into the lungs. Ideally the child should be under NPO (no per oral) I.e. no food and water for min 6-8 hours. This is the responsibility of the anaesthetist

    I agree many fakes are resideing in karnataka.. Doctors/Engineers etc.
    But most of them do stay in cities like Bengaluru for sake of money. Peple shud aviod going to small clinics when it comes to Babies and Elder people. We need to get out treatment done with trutsed doctors or Medical Institutions.

    It is absolutely sad to note the tragic demise of the small kid and the pain of parents. But, taking the law into hand by the relatives of the child and beating the doctors is totally unlawful. This is becoming very common in our country. This will lead to a situation wherein even in case of emergency Doctors definitely refuse to conduct surgery or treatment for fear of getting beaten up. Where are we leading to. If the Doctor commits a mistake out of negligence, let the law take a decision but not the relatives of the patient. This is really barbaric. Let us uphold the civic discipline.

    There is no clarity if the Child was intubated and crisis began under GA. seems to be like an anaphylatic shock and thereafter very poor emergency management. should have been alert in shifting to the best pediatric ER without hesitation atleast.

Presently most of these medical people get in to college by paying huge money, and complete the course with out much practical exposure or proper guidance.     I am 31 yrs. I also a tooth (Last tooth in my jaw) problem with a minute hole that had been causing pain. I went to doctor to fill it up at Tripunithura. After filling the tooth broke when i started eating hard items. Then i went to Kottayam Medical Center to rectify they suggested Root Canal Treatment and a days rest, instead my sister a GNM nurse suggested removal of tooth to avoid frequent travel to Kottayam due to her past experience with root Canal treatment which involved 5 visit to dentist. But i had difficulty removing with intense pain as it was last teeth with anesthesia which froze my jaw for a longtime atleast 2-3hrs. Lastly i withdrew, and decided to retain the tooth. I started using Colgate white toothpaste and Sensodyne toothpaste together and i have no more tooth ache even though tooth is half broken. I think for a child of milk tooth root canal is disastrous with anesthesia. Just a proper wash and removal of dirt would have been enough.

    My suggestion to all-
    always take second opinion & the best hospital with best doctors when required to perform surgery or anesthesia to be administered to small kids(in an emergency case there will be no time for this).
    Apart from cardiac arrest, their brain might lose some performance after revival( side effects).
    Beating doctors is no solution as well. No doctor wants his patients death in his clinic through negligence.

    In each District Head Quarter there should be a branch of Indian Medical Council with a committee of doctors to issue permission letter to conduct Surgery. Surgery should be conducted at any hospital prior permission of the council so that unnecessary, treatment, surgery etc.will reduce.

    It needs to be established whether an overdose happened and if so, the strongest punishment for negligence should be meted out. But whether or not that was the case, why were the doctors waiting when they knew the child had been unconscious for so long? They could have saved a child's life. How is the brother of the dentist so sure that there were three cardiac arrests - isn't that for a post mortem report to decide? And if the dentist himself thought that was the case, why was he wasting time by shifting the boy to the ward instead of moving the child for life saving procedures?
    The medical and police authorities need to wake up, kids can't die like insects.

    The most likely possibility is anaphylaxis reaction to the local anaesthetic which can happen to anyone and it doesn't depend on the amount given , instead any small dose can cause that .if the child was given emergency drug immediately when they saw the reaction then pt can survive otherwise leads to cardiac arrest.
    I don't think we can blame the dentist instead I can blame the system as they don't even teach the postgraduate students how to deal with medical emergencies in the clinic!

    Removing the tooth of a 3 year old also requires anesthesia. It is not the treatment of choice. The right treatment is pulpectomy and save the tooth.

    Some negligence by doctors.Even though Over dose of anesthesia/ cardiac arrest or aspiration of vomitus if doctor have had given Basic life support like intubation,medicine and immediate referral to higher centre the child might have survived.

    definitely a case of gross medical negligience on the part of the dentist..surely it appears that it could be that the dose could have exceeded or not injected.. sad that a cute and small child had to die in this manner .the concerned hospital and the dentist in particular should be barred from carrying on their profession.. if the police is not able to do this then the local people should boycott the hospital.

    Looks likes negligence by doctors.If the child would have been treated conservatively for tooth ache instead of filling the cavity, the child would have been saved. Hasty decission has caused a life loss. Poor parents lost their child at the budding age. Dental council has to take stern action against doctors who dont follow the guidelines in running dental clinic.     I agree with KRPrabhu. We often discuss about negligence from Doctors. But, it is important that parents too should have some knowledge. If the child had past history of cardiac problems, it is prime duty of parents to inform the doctor about it. It is also duty of doctors to ask the parents about any past medical history before administering any medicine. Very sad and heart breaking news.

    People who are having past record of various ailments,especially those who have heart problem/undergone heart sergeory,while availing dentists services should clearly inform him the past history and also drugs they are taking,any elergy for certain drugs etc., to avoid complecations later stage.In the given case child of three yrs should not have taken to dentist fo treatment so early.Medical negligence is matter every where now a days.......Very sad incident...RIP....
